1. Open Google Chrome Browser

2. Sign in to your Google Account using your Email ID and Password

3. Go to Google Apps and click on Forms

4. Or You can visit directly to Google Forms Website, i.e., https://docs.google.com/forms

5. Once the site is loaded, the following screen displays

6. Click on + from blank to create a new form or if you want to make a Google Form from scratch, choose the option “Template Gallery.”

7. In the form box, you can add multiple questions with multiple options.

8. On the Google Forms Creation Page, you can update the following details

  1. Click on + to Add a Question.
  2. Click on -> option to import questions from saved forms
  3. Click on TT to add a title and description
  4. Click on the image icon to insert the image into the Google Form
  5. Click on the video icon to inset Video to the Google form
  6. Click on the Section icon to add the section for form
  7. Click on Customize Theme to customize the Forms with a Header image, Background color, etc.
  8. Choose the settings option to make the settings
